<p>New Graduates from High School / Transferees / Graduates of other courses:</p>
<ol>
<li>Must have the general average of 85% in high school as evidenced by the original report card (Form 138).</li>
<li>Must have a general average of at least 2.0 or its equivalent grade for transferees / graduates of other courses as evidenced by an original copy of transcript of records.</li>
<li>Must have passed the Mental Ability Test with a cut-off score of 30 points.</li>
<li>Must take the English College Entrance Test with a cut-off score of 34 points. (A score 34 points and above shall proceed to English 101, whereas 33 points and below must take English 100).</li>
<li>Good Moral character certification by HS Principal / Guidance Counselor.</li>
<li>Copy of NSO – authenticated Live Birth Certificate</li>
<li>Copy of Marriage License (if married)</li>
<li>Must be physically and mentally fit</li>
</ol>
<p>­    *   Must have the Nursing Aptitude Test with a raw score of 95   points and above to be given Second Semester of the Second Year.</p>
<p>­     *  Must submit recent original result of the following diagnostic test: Chest X-Ray, Hepatitis b screening test, complete Blood Count, Fecalysis and Urinalysis to the School Physician for evaluation. (The mentioned diagnostics test be conducted at Capiz Emmanuel Hospital, the affiliate hospital of the College). Request form must be obtained from the School Clinic only.</p>
<p>­     *  Applicant with physical defects should undergo further screening and evaluation; however, the College of Nursing has the right to refuse admission.</p>
<p>­      * A married applicant will be accepted provided he/she must conform to the following requirements:</p>
<p>* (a)     Submit a certified photo copy of the marriage contract.</p>
<p>* (b)     Should not get pregnant during the entire study, if possible. Waiver must be signed by both husband and wife. However, in case she gets pregnant, she will be advised to be on leave for one (1) year and she may continue the following year after her delivery.</p>

<p class="MsoNormal" style="text-indent:-.25in;text-align:justify;margin:0 0 0 .5in;"><span style="font-size:10.5pt;font-family:Arial;"><span>b)<span style="font:7pt &quot;">      </span></span></span><span style="font-size:10.5pt;font-family:Arial;">The Dean and / or Department chairperson will be furnished by the <span style="text-decoration:underline;">Registrar&#8217;s Office with the list of students</span> with “incomplete” grades, who have been issued transfer credentials. The names of the teachers concerned will be included in this list.</span></p>
<p class="MsoNormal" style="text-indent:-.25in;text-align:justify;margin:0 0 0 .5in;"><span style="font-size:10.5pt;font-family:Arial;"><span>c)<span style="font:7pt &quot;">      </span></span></span><span style="font-size:10.5pt;font-family:Arial;">The Dean/Department chairperson notifies the teachers concerned that the students in the list cannot be allowed to complete work to “remove” their “incomplete grades,” in compliance with par. 2C circular 9, 2. 1963.</span></p>
<p class="MsoNormal" style="text-indent:-.25in;text-align:justify;margin:0 0 0 .5in;"><span style="font-size:10.5pt;font-family:Arial;"><span>d)<span style="font:7pt &quot;">      </span></span></span><span style="font-size:10.5pt;font-family:Arial;">If teachers are still correcting test papers at the time the student requests a transfer credential, the student will be asked to go to his/her dean who asks his/her teachers to certify whether or not the student may have to be given “inc.” grades.</span></p>
<p class="MsoNormal" style="text-indent:-.25in;text-align:justify;margin:0 0 0 .5in;"><span style="font-size:10.5pt;font-family:Arial;"><span>e)<span style="font:7pt &quot;">      </span></span></span><span style="font-size:10.5pt;font-family:Arial;">If one or more teachers indicate an “incomplete grade” for the student but the latter insists that he/she be issued a transfer credential his/her incomplete grade notwithstanding, item <span style="text-decoration:underline;">a</span> and <span style="text-decoration:underline;">b</span> above, will be followed. If no “incomplete” grade is indicated for the student, a T/R may be issued with the list of subjects, with or without grades. Another T/R may be issued later with all the grades in.</span></p>
<p class="MsoNormal" style="text-indent:-.25in;text-align:justify;margin:0 0 0 .5in;"><span style="font-size:10.5pt;font-family:Arial;"><span>f)<span style="font:7pt &quot;">        </span></span></span><span style="font-size:10.5pt;font-family:Arial;">A teacher should prepare as many copies of the Completion Report as there are colleges represented in the completion list unless he/she prefers to prepare a completion report for each college concerned. (If all deans concerned get copies, teachers are not likely to prepare completion grade for a student more than once. This is one explanation given for the fact that the Registrar&#8217;s Office gets more than one completion grade for a student. In a few cases, there were discrepancies in the grades given to a student.)</span></p>
<p class="MsoNormal" style="text-indent:-.25in;text-align:justify;margin:0 0 0 .5in;"><span style="font-size:10.5pt;font-family:Arial;"><span>g)<span style="font:7pt &quot;">      </span></span></span><span style="font-size:10.5pt;font-family:Arial;">Some teachers are forced to give failing students “Inc.” because these students do not take the final exam whereas students who may have done slightly better work are given 5.0 outright because they take the final test and fail it.</span></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><span style="font-size:10.5pt;font-family:Arial;"> <span id="more-134"></span></span></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><span style="font-size:10.5pt;font-family:Arial;"><span>            </span>To preclude this irregularity, the VP-AA Office should have <span style="text-decoration:underline;">prepared forms</span> to be used by teachers to remind students to take a scheduled special exam. The reminders inform the student that he/she will be given 5.0 if by the middle of the term following the term of enrollment he/she will not have taken the specially schedule final exams.</span></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><span style="font-size:10.5pt;font-family:Arial;"> </span></p>
<p class="MsoNormal" style="text-indent:-.25in;text-align:justify;margin:0 0 0 .5in;"><span style="font-size:10.5pt;font-family:Arial;"><span>h)<span style="font:7pt &quot;">      </span></span></span><span style="font-size:10.5pt;font-family:Arial;">To inform students and their deans of “<span style="text-decoration:underline;">inc</span>” grades, <span style="text-decoration:underline;">each faculty member will make a list of students</span> (in each of his/her subjects NOT classes) who obtained a grade of “Inc.” and make as many copies of this list as there are colleges represented in a list. <span style="text-decoration:underline;">Each dean</span> posts all the list received by his/her unit on his /her college bulletin boards, for the information of all.</span></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><span style="font-size:10.5pt;font-family:Arial;"> </span></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><span style="font-size:10.5pt;font-family:Arial;"><span>            </span>So, students must consult their dean&#8217;s bulletin boards a few days after final exam time and must contact the teachers concerned, regarding completion requirements. The “Inc.” grades not removed within the <span style="text-decoration:underline;">one-year<span>  </span>grace period</span> as well as those entered in official transcripts of records (Valid for transfer) cannot be removed.</span></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><span style="font-size:10.5pt;font-family:Arial;"> </span></p>
<p class="MsoNormal" style="text-indent:-.25in;text-align:justify;margin:0 0 0 .5in;"><span style="font-size:10.5pt;font-family:Arial;"><span>i)<span style="font:7pt &quot;">        </span></span></span><span style="font-size:10.5pt;font-family:Arial;">Teachers submit a list of “Inc.” to deans before the enrollment period.</span></p>

We are the members of the Work Study Program organization where in we study and work at the same time. To be a working  student is not an easy task. Being a Library student assistant, you have to be a responsible and a hardworking person. Responsible in the sense that, you have to render your duty on-time. Be approachable to all students eventhough you are not in the mood. A hardworking in the sense that you have to bear in mind that you have to work hard to sustain your study.</p>

I acquire new friends here. i&#8217;ve learned to act as a proffessional. Library molded me to become a better person, at the same time. To be strong enough to face all the challenges that comes in my life . I know that, after my college years the things that I&#8217;ve learned here, i will apply that in my own field. I am thankful and blessed to have a supervisors that understands and give me a pieces of advice. Comes time that, we will apart from each other, and i never forget all those advices that you&#8217;ve impart to me. Those will be my guide to achieve all my plans and goals in life.
